Accessible through IrisOC, our community app for older adults in Orange County, this interactive assessment facilitates the transfer of lifestyle information from patient to provider. For efficiency, it can be printed or emailed, giving doctors valuable insights before a patient visit.
Designed with ease-of-use in mind, the interface features large buttons and clear, easy-to-read text. It helps reduce the resistance to technology often presented by older users, making the experience more intuitive and stress-free.
